With the lack of health care options and even limited internet access in many rural areas, nurse-first triage is helping address social determinants of health in these communities. Every time a patient calls, they speak with a nurse and receive the right level of care when they need it, regardless of where they are located. While nurse triage was making an impact years before the pandemic, COVID-19 further demonstrated how this access point can improve care delivery. Cheryl Dalton-Norman just celebrated 40 years as a nurse and also serves as President of Conduit Health Partners, which she co-founded. Conduit works with health systems, health plans and employers to connect patients to much-needed care in more than 350 locations across 48 states.
